The Commission for Nurse Reimbursement calls for a fundamental shift: separating nursing care from hospital room charges and billing it directly based on actual services delivered.

For nearly a century, nursing services have been bundled into daily room rates, masking the true value and variability of direct patient care. This outdated model drives staffing cuts, worsens patient outcomes, and fails to reflect the essential role nurses play in high-quality, value-based care.

  • No price = no value: Nursing care is excluded from hospital chargemasters, DRG cost weights, and CMS price transparency requirements.
     
  • Hidden costs: Hospitals save millions by reducing nursing hours—without lowering room rates—resulting in care that payers reimburse but patients never receive.
     
  • Quality at risk: Reduced nurse staffing is directly linked to higher mortality, longer hospital stays, and adverse events like infections and falls.

  • Bill nursing care separately using existing billing codes (e.g., NUBC 023x) based on actual direct care hours.
     
  • Modernize DRG reimbursement with a Nursing Case Mix Index (NCMI) to align payment with nursing intensity.
     
  • Create a national nursing care database to support quality metrics, clinical outcomes, and value-based payment models.

Statement of Purpose

The Commission for Nursing Reimbursement was launched in response to nationwide concerns about nursing staffing shortages. It will work to change the way that Medicare reimburses healthcare systems for the valuable care that nurses provide.


The Commission aims to evaluate, review, and recommend improvements to the Centers for Medicare and Medicaid Services on how to allow for payment for nursing services in hospitals and other healthcare settings.


The Commission will work alongside nursing organizations and partner with healthcare groups across the nation to propose a solution to Congress that takes into account the needs of the more than 5 million nurses in America today and seeks to reduce the high cost of quality healthcare. We will secure bi-partisan support to improve the sustainability and stability of the nursing workforce.

Our Mission

The Commission for Nurse Reimbursement (CNR) was founded in April 2023 by Rebecca Love and Sharon Pearce with a clear mission: to transform how nursing care is valued and reimbursed. Our goal is to address the critical financial challenges that have contributed to the nursing shortage and, ultimately, improve patient care.

We believe that nurses are the backbone of the healthcare system and that their invaluable contributions should be recognized financially. By advocating for nursing care to be viewed as a billable service rather than an expense, we aim to elevate the profession and ensure its sustainability for the future.

Nurses are present in every congressional district across the country, and their voices deserve to be heard. We will leverage this grassroots power to advocate for policies that support nurses and improve patient care.
